The U.S. Chemical Safety and Hazard Investigation Board announced that it plans to send a six-person team to Louisville after starting an “initial” probe last week. <\/p>\n
Steve Owens, the board’s chair, said an investigative team is being deployed to the scene to conduct formal interviews, document the incident site, and gather important evidence to help ensure that a tragedy like this does not happen again. <\/p>\n
The board, known as CSB, previously investigated the 2003 blast at the Payne Street facility, concluding that a vessel called a feed tank most likely failed after it overheated and exploded. One worker was killed.<\/p>\n
The plant at the time was operated by D.D. Williamson & Company, which was acquired by Givaudan in 2021.<\/p>\n
Last week’s explosion, which killed two employees and injured 11 others, was caused by a cooking vessel that overheated and exploded, according to preliminary findings from the Bureau of Alcohol, Tobacco, Firearms and Explosives. The ATF’s Louisville field office and the Louisville Fire Department led the initial investigation into the cause.<\/p>\n
The Kentucky Labor Cabinet, which oversees workplace safety standards in the state, also has opened an investigation, which is expected to take up to six months to complete.<\/p>\n
The Kentucky Public Protection Cabinet, through its Department of Housing, Buildings and Construction, also is investigating, spokeswoman Kristin Voskuhl said in a statement. She said the department is offering expertise on state regulations related to the installation of boilers, pressure vessels, and pressure piping and committed to assisting the investigation team in uncovering the root causes of this tragic and catastrophic event.<\/p>\n
CSB does not levy citations or fines but makes safety recommendations. In its previous investigation, the board found that D.D. Williamson did not have a number of safety programs in place and recommended that the company ensure that all vessels have adequate overpressure protection that includes alarms, among other things.
