Elderly residents of Hart, Metcalfe, and Barren counties will now be able to receive health care at home.
The University of Louisville Foundation is getting a $2.55 million dollar grant from the Department of Health and Human Services to provide care for individuals 65 years and older who have two or more chronic conditions.
Some of that money will go through T.J. Samson Hospital in Glasgow to help provide teams of medical personnel to go to patients’ homes to deliver the comprehensive care they need.
The grant is coming from the Health Resources and Services Administration to bring health care to older rural Kentuckians who have not been getting the medical attention they need.
