
The Kentucky Board of Pharmacy has issued an order that a Henry County pharmacy stop compounding and dispensing compounded medications after three children had an adverse reaction to bad medication.
The Kentucky Board of Pharmacy investigated Med Save Pharmacy in Eminence after an alleged medication error led to the hospitalization of three children who were given a compounded clonidine oral suspension.
The board found that the pharmacy was not in compliance with the FDA recognized United States Pharmacopeia standard in several areas including a lack of documented training of pharmacy personnel, appropriate pharmacist oversight and missing or inappropriate documentation for compounding records.
Therefore, the board met for an emergency meeting on Wednesday (08/38) and requested an order requiring Med Save Pharmacy to cease all compounding and dispensing of compounded medication immediately. The order was signed by the pharmacy that day.
The Kentucky Board of Pharmacy said in a statement it will continue the investigation into the alleged error that led to the three children being hospitalized. The initial investigation has revealed that the alleged bad medication was only dispensed to the three children and that the incident is isolated to the Henry County pharmacy.
The Kentucky Board of Pharmacy said it holds the pharmacy accountable for practicing outside the standard practice for a compounding pharmacy.
